Data Subject Application Form
- DATA SUBJECT APPLICATION FORM GENERAL EXPLANATIONS
Under the Law No. 6698 on the Protection of Personal Data (“KVK Law”), individuals defined as data subjects (hereinafter referred to as “Applicant”) have the right to make certain requests regarding the processing of their personal data, as specified in Article 11 of the KVK Law.
In accordance with the first paragraph of Article 13 of the KVK Law, applications regarding these rights must be submitted in writing to our Company, which is the data controller, using the following methods determined by the Personal Data Protection Board (“Board”).
In this context, applications to our Company that are to be made in “writing” should be submitted by printing out this form and:
- By the applicant’s personal submission,
- Via a notary,
- By registered mail with return receipt,
- By being signed with a “secure electronic signature” as defined in the Law No. 5070 on Electronic Signatures and sent to our registered electronic mail address.
Below, information is provided regarding the methods for submitting written applications. If the methods listed below are not followed, we will not respond to incorrect applications if identity verification cannot be completed or if there is suspicion of identity fraud by malicious third parties. The responsibility for applications that are incorrect or incomplete rests with the applicant, and as part of our obligation to protect personal data under the law, our company cannot share information with unidentified or malicious third parties.

In accordance with the second paragraph of Article 13 of the KVK Law, your applications received by us will be responded to within thirty days from the date we receive your request, depending on the nature of the request. Our responses will be provided to you in writing or electronically, as stipulated by Article 13 of the KVK Law.
